Registration
A new school enterer is defined as: any child between the ages of two months and 21 years entering or being admitted to a Arizona school district for the first time, including but not limited to, foreign exchange students, immigrants, students from other states and territories, and children entering from non-public schools.
Registration Requirements
- Students entering kindergarten must turn 5 years of age by August 31. The following may be used to verify age only:
- Birth Certificate - with raised seal
- Passport/Visa
- Hospital Certificate (if it states the parents' names along with the child's name and date of birth).
- Parent's Affidavit for students coming to Arizona from out-of-state - birth certificate with raised seal must be presented within 60 days of registration.
- Students coming from private kindergarten or out-of-state kindergarten who do not meet the age requirement for first grade will be placed in kindergarten.
- Child must reside with the parent, legal custodian, guardian or relative caregiver - the child must be legally under the care of the adults with whom they reside to attend a Brandywine School District School.
- Person registering must be the legal custodial parent. Custody documents may be necessary to establish the legal guardian and to determine residential custody.
- Power of Attorney is not acceptable
- Family Court Petitions are not acceptable
- Interim Family Court Order for guardian is accepted for registration - however, Final Court Order must be presented to the school within 60 days of registration.
- Notarized statements are not acceptable*
* - A stepparent is allowed to register for the legal parent IF a notarized letter from the legal parent is given and includes the names of the stepparent and child. Also, a copy of the marriage certificate must accompany the notarized letter from the legal parent for additional proof that person registering is the stepparent.
- Court documents must be dated, and signed by a Commissioner, Judge or Master. These documents will be copies only - original retained in the Court House.
- Any student seeking admission to Gila County Schools may be required to provide information regarding expulsions, arrests, which may have resulted in a formal charge and any involvement with the Juvenile Justice System, at the time of initial registration.
- Legal parent/guardian must provide proof of identify (for identification purposes only - cannot be used for proof of resident).

- AZ Motor Vehicle ID card
- Arizona Health and Social Services ID card
- Passport
- Permanent Resident Card
NEW Residents
- The parent, legal custodian, guardian or relative caregiver must provide the following documentation or information:
- Birth Certificate - with raised seal - with parent's name on the birth certificate.
- If birth certificate does not contain the parent's name who is registering, additional guardianship verification is required.
- A legal document may be provided if it states the parent's name, relationship to child and the child's date of birth.
- Passport - in lieu of a birth certificate.
- # Home telephone number. If using a cell phone as a home phone number, invoices from the previous two months must be submitted showing the home address with the cell phone number. However, if the resident has no home telephone or cell phone, the child will not be excluded from school.
- The resident must provide a notarized statement that they have no telephone number and must list the name and phone number of an emergency contact.
- Child's Social Security Number - child will not be excluded from school if this information is not provided.
- Immunization record - must state month/year of each immunization
- DtaP, DPT or DT - 5 or more doses unless 4th dose was given after the 4th birthday.
- # OPV or IPV - 4 or more doses unless 3rd does was given after the 4th birthday
- MMR - 2 doses - first does should be administered on or after the age of 12 months. The second does should be administered after the 4th birthday.
- Hepatitis B - 3 doses
- Varicella - Varicella vaccine, Varicella lab evidence or documentation that the student has had the disease. Beginning in the 2003-04 school year with kindergarten then adding the grade for each subsequent year.
- Physical examination within the past 24 months.
- Mantoux Tuberculin Skin Test or Pediatric TB Risk Assessment with results reported within the past 12 months.
- Evidence of Lead Blood test - Documentation for children age 5 or younger.
- Transcript from previous school along with the withdrawal form.
- CPS placement into foster home:
- Student's birth certificate
- Social Security number, if known
- Updated shot records
- Placement verification into foster setting
- CPS registers the child
School - Open Enrollment
In Arizona families are fortunate to have the Arizona Open Enrollment Program that allows parents the opportunity to enroll their children in the school system that will most benefit them.
Gila County Schools are an open enrollment schools. If a student resides out of the boundaries of a school district, an open enrollment form must be completed. An open enrollment form may be obtained from any school in the district. If parents want to know by June 1 if the student is accepted at the school, the open enrollment form must be submitted to the school by April 15th. If an open enrollment form is submitted after April 15, parents may not be informed until the first day of school.

Kindergarten Programs in GIla County
There are a variety of Kindergarten programs in Gila County.
- Regular 1/2 day kindergarten at each K-4 Elementary School
- Full Day Kindergarten (FDK) where parents pay for the extra half day of kindergarten on a sliding scale based on income as verified through the free/reduced lunch program at each K-4 Elementary School.
- Extended Day Kindergarten which provides an extra 1/2 day of kindergarten for identified academically at-risk students and begins shortly after the school year begins and students are tested.
- Title I At-risk Kindergarten at
- Special Education Kindergarten at Copper Rim for handicapped and some typical students.
All Kindergarten students are scheduled for assessment under Child Find procedures when they register for school. The assessment tool used is the AGS Early Screening Profiles, “a nationally normed screening battery,” used to identify those “with possible handicaps or other problems that may interfere with development.”
By 2004-2005 school year all kindergarten classrooms across the district will be implementing the same curriculum, through the Breakthrough to Literacy computer assisted program which is aligned with the Arizona Standards, the Harcourt Brace basal series being adopted across the district in grades 2–5, and an approved core program for NCLB Reading First programs.

State Regulations on Administration of Medications
The following is intended to give parents a better understanding of the State Board of Education's regulations concerning the administration of medications to students in all schools in the State of Arizona.
ADMINISTRATION OF MEDICATIONS
Any medication which has been prescribed by a physician can be administered by the school nurse. The school nurse may also administer non-prescription medications:
- The medicine must be in the original container (State Law)
- The medicine must be accompanied by a note signed by the parent stating:
- the time the medicine is to be given in school;
- the reason the medicine is to be given;
- the allergies (if any) that the child has, particularly to medications; and,
- the telephone number where the parent can be reached during the day.
- If the nurse has any questions about the administration of the medicine, she may withhold the dispensing of it until she has been able to contact the parent.
